Helping youth transition
to adulthood
The transition from youth to adulthood is particularly important and can be complex. Community Living Dryden-Sioux Lookout offers support to assist in facilitating the transition from high school and/or children’s services to adulthood. Our Intake Coordinator can provide support to youth and their family/support network to plan for their future and connect them with the appropriate community resources; members, agencies, programs and funding applications.
We will collaborate with the youth, their family, school representatives, agency representatives and other support personnel to assist them through the transition planning process. They will draw upon the strengths and contributions of each team member to ensure a coordinated effort. The Intake Coordinator will support the team and youth/family to accomplish the outcomes set out in the transition plan.
