WE’LL GET YOU INVOLVED
IN COMMUNITY ACTIVITIES
The Community Participation Program offers an individualized community based experience. People who participate have the opportunity to experience a variety of leisure, volunteer and recreational activities based on individual interests and choices in the community with friends of their choice.
YOU’VE GOT GOALS
WE’RE HERE TO HELP YOU REALIZE THEM
Personal Outcomes Management System (POMS) is how we keep track of your goals.
Our Personal Outcomes Management System organizes goals in these areas but you choose what your goals are:
- Health and Wellness
- Community Presence and Participation
- Independence, Choice and Control
- Relationships
- Learning
